A prescription and completed admissions forms are required. Onsite physicians assess whether treatment is appropriate. Sessions are by appointment only.

Arrive 10 minutes early. The entrance is at 59th and Hollis Streets, with designated HH parking in the building lot. The clinic describes wheelchair access, a Hoyer lift and staff assistance from your vehicle.
