Planning your visit

New patients are asked to arrive 30 minutes before their first appointment, park in the hospital lot and enter the 33 building on the main campus; the center is on the first floor. Patients should bring the physician order and any related forms from their physician, an insurance, Medicaid or Medicare card, photo identification, their medications or a list with doses and timing, a list of allergies, and films or reports from studies done elsewhere. Patients should check with their insurer about authorization requirements. Patients coming from nursing homes or other extended-care facilities must be accompanied by a family member who can provide the necessary information. No special preparation is needed unless the physician says otherwise. Appointments are scheduled by phone; a voicemail should include the patient's name, birthdate, phone number, physician's name, diagnosis and insurance company. Written instructions are provided at the end of the visit.
