Planning your visit
Free parking is available outside the Cumming building. Enter building 1505, continue to the end of the hallway, then turn right to Suite 1300 on your left. The clinic currently posts a construction access map; check it for updated directions before your visit. Complete the online patient forms before your first visit; they are sent directly to the center and do not need printing. Bring a photo ID, insurance card and any applicable advance directives. Your treatment plan follows an evaluation. During treatment you can watch television, listen to music or nap.
