Planning your visit

The clinic publishes a patient intake form and an insurance referral form. For insurance referrals, its checklist requests a copy of your driver’s license and insurance card, relevant physician progress notes, scans or X-ray reports, and a recent HbA1c result if you are diabetic; HMO patients should include their primary-care contact information. Ask your referring office to coordinate these records. HBOT sessions usually last 60–90 minutes. Contact the clinic with questions about the forms or your appointment.
