Planning your visit
Book a consultation by phone or online. Bring your ID and insurance card to every visit, a complete medication list including over-the-counter medicines, and a glucose log if you have diabetes. The clinic publishes patient registration and consent forms online. Its patient brochure asks for 24 hours’ notice for cancellations or rescheduling; patients arriving 15 minutes or more late may be asked to reschedule.
