Planning your visit
The office provides a required new-patient health-history form online; print and complete it, then fax it to the office or bring it to the appointment. Its published first-visit guide is for chiropractic care: a consultation reviews concerns and goals, followed by an examination and X-rays if needed. The doctor discusses findings, treatment recommendations and whether another provider is needed. Nothing is performed without consent, and you may discuss concerns during treatment.
